WHAT IS PROMISE?
PROMISE: MINING INBORN ERRORS OF IMMUNITY FOR PERSONALIZED THERAPEUTIC STRATEGIES IN NON-MONOGENIC INFLAMMATORY DISORDERS
Mutations in genes that regulate immune signalling and differentiation pathways can cause inborn errors of immunity (IEI), leading to recurrent infections, autoimmunity, inflammation, and cancer.
Although more than 450 genes have been identified, their impact on clinical symptoms and treatment responses remains only partially understood.
PROMISE takes a multidisciplinary approach to extend pathway-oriented personalised medicine strategies from monogenic IEI to complex immune-mediated inflammatory conditions.
It builds on omics-based patient categorisation, using monogenic IEI as a reference, to identify biological similarities and guide mechanism-based repurposing of immune-modulating drugs. PROMISE combines clinical expertise, molecular diagnostics, bioinformatics, and multi-omics technologies to establish disease signatures, study treatment responses, and develop personalised therapeutic strategies for patients with complex chronic inflammation.

On May 1st, 2025, the PROMISE project was officially launched. This new consortium brings together clinical scientists, epigeneticists, bioinformaticians, data scientists, and a molecular diagnostics SME to advance personalised medicine for patients with complex immune-mediated inflammatory conditions.
IMPACT
PROMISE aims to improve the lives of people with IEI and complex inflammatory diseases by creating better diagnostic tools and more personalized treatments. These advances could help more than 10 million patients and speed up finding the right treatment by 5–10 years.
How PROMISE Will Make a Difference:
Better patient care: We identify biological markers that help doctors understand each patient’s disease and choose the most effective, targeted treatment. We are also developing a new tool to diagnose and monitor these conditions earlier and more accurately.
Lower healthcare costs: Early diagnosis of immune disorders can already save thousands of dollars per patient. PROMISE will reduce total costs by an additional 20% by cutting misdiagnoses and ensuring patients receive treatments that work best for them.
Improved quality of life: By reducing delays in diagnosis and providing personalized therapies, PROMISE aims to significantly improve the daily lives of patients and their families.
